| The Bereavement Coordinator develops, coordinates and evaluates a comprehensive bereavement program for those patients, caregivers and families of the Hospice program that are dealing with terminal illness and/or death. The Coordinator provides individualized, caring support during the grieving process and teaches practical coping skills through ongoing bereavement counseling. This is a part time 5-10 hour per week position. |
Duties: |
| The chosen candidate will perform spiritual assessment and identify individual counseling needs for patients/families and caregivers of the Home Health/Hospice program. Responsible for coordinating the delivery of professional bereavement services through home visits, phone calls, mailings and/or referrals. Provides education to the interdisciplinary team regarding the grief process and related issues. Documentation of care provided and the response of the patient/family according to policy. Stays abreast of predoeminate religious/spirtiual systems in order to best meet the needs of a diverse population. Will be responsible for coordinating community outreach activities as well as facilitate bereavement support groups. |
Qualifications: |
| Master's degree in a related area, such as divinity, social work, theology or thanatology. GC-C certification through the American Academy of Grief Counselors is preferred. At least two years of experience in hospice, counseling or other areas dealing with grief and bereavement. Selected candidate must have strong assessment skills and have the ability to teach people with varied learning styles as well as prepare individualized counseling plans. The ability to organize and facilitate bereavement support groups necessary. Must have knowledge/understanding of predominate religious/spiritual systems. Excellent written and verbal communication skills desired. Ability to exercise independent decision making and exercise discretion in confidential matters is essential. |
